Should any additional information be received by the customer then an additional report will be submitted.(b)(4).Device evaluation: the reporter of the incident stated that it is was discovered through an internal investigation that the nurse bumped into this unit causing a disconnect of the device, ultimately resulting in the dumping of pressure and the oscillator stopping.A carefusion field service representative went onsite to the reporter's facility and evaluated the unit.The unit passed inspection, meeting manufacturer specifications.There was no failure detected.
 
Event Description
The customer reported that this 3100a high frequency oscillating ventilator was being used on a patient when it unexpectedly stopped oscillating.The customer stated that alternate ventilation was provided by placing the patient on a conventional ventilator and there was no patient injury or harm associated with this event.
