Legal correspondence was received.It is alleged the patient suffers from squeaking, popping, dislocation and infection.

Event Description
Update ¿11/21/2016 medical records received.After review of the medical records for mdr reportability, in addition to what was previously reported, the revision surgery notes reported no clinical evidence of infections.The acetabular component and femoral component were well fixed.Significant metallic staining of the soft tissue was reported and the poly liner was noted to be displaced with failure of the locking mechanism and the locking nipples were sheared off the component.

Update 01/19/17 ¿ medical records received.After review of the medical records for mdr reportability, the revision operative note indicates that the polyethylene liner clearly disassociated from the cup.Disassociation harm and fda code added to liner and cup.Prod/lot corrected and updated for all products.

Pfs and medical records received.Pfs alleges pain, injury, popping sensation and heard a metallic screeching.After review of medical records for mdr reportability, patient was revised to address failed right total hip arthroplasty with polyethylene component.Revision notes reported significant metallic staining of the soft tissues, failure of the locking mechanism with polyethylene, locking nipples were sheared off, and a blood-tinged fluid within the periarticular space.The cup and stem were not revised.There were no lab results provided.
