(b)(4).The actual device was returned for evaluation.Reliability engineering evaluated the device and determined that the device did not function and had no power.It was further determined that the trigger was sticking.It was observed that the electric motor was damaged and not functional.It was further observed that there was an unknown liquid found on the internal components and the seals were worn and damaged.Therefore, the reported condition was confirmed.The assignable root cause associated with the trigger sticking was due to component wear and the unknown liquid and seal issue was due to improper cleaning/care, which is user error, misuse, and / or abuse.If additional information should become available, a supplemental medwatch report will be submitted accordingly.

It was reported that the battery reciprocator device would not power up at all.According to the reporter, the ¿bad reciprocator¿ device damaged the battery device.During in-house engineering evaluation, it was observed that the trigger was sticking.The event was not reported to have occurred during surgery.It was not reported if there were any delays in a surgical procedure or if a spare device was available.The reporter stated that there was no patient incident reported.There were no reports of injuries, medical intervention or prolonged hospitalization.The exact date of this event was unknown.All available information has been disclosed.If additional information should become available, a supplemental medwatch report will be submitted accordingly.
