(b)(4).Age at time of event: 18 years or older.(b)(4).Device evaluated by mfr: the device was returned for evaluation.Device analysis noted that the guidewire was broken at 207 cm from the distal section.The spring tip was not returned.The overall length and the outer diameter (od) of proximal section were not measured due to the device condition (wire broken in the middle of the device).The od of the distal tip and the middle of the device near the broke section is within specification.The manufacturing batch record review confirmed that the device met all material, assembly and performance specifications.The root cause is not confirmed as there was no evidence of the alleged issue or any anomalies which could have contributed to the reported difficulty.(b)(4).
 
Event Description
Reportable based on device analysis completed on 30aug2016.It was reported that a guidewire kink occurred.The 90% stenosed target lesion was located in the moderately tortuous and severely calcified left anterior descending (lad) artery.A rotawire¿ and wireclip¿ torquer was selected for use.Upon preparation, outside the patient's body, it was noticed that the rotawire was kinked.The procedure was completed with another of the same device.No patient complications were reported and the patient's status is good.However, device analysis revealed the wire was broken in the middle of the device.
