Event Description
The patient (via the manufacturer's representative) reported that they could not charge the implantable neurostimulator (ins) effectively and he thought the ins was flipped in the pocket.The patient also reported intermittent stimulation issues, indicating that he thought the ins was turning off and on by itself without the patient charging or using the patient programmer.The manufacturer's representative interrogated the ins on (b)(6) 2016 and found normal impedances.During interrogation, there was a short lived interruption in connectivity between the physician programmer and ins.The ins was replaced on (b)(6) 2016 and the issue was resolved.It was noted that impedances were checked intraoperatively during the replacement and were normal.The indication for use was spinal pain.

Addition information from the manufacturing representative indicates that the explanted ins has been returned.
