It was reported that prior to surgery, it was observed that the bearings fell out fo the attachment device.It was not reported whether there were any delays in a planned surgical procedure or whether a spare device was available for use.There was no patient involvement reported.There were no reports of injuries, medical intervention or prolonged hospitalization.All available information has been disclosed.If additional information should become available, a supplemental medwatch report will be submitted accordingly.

The actual device was returned for evaluation.Reliability engineering evaluated the device and the reported condition was confirmed.It was determined that the device had missing ball bearings, spacer and bearings spacer.The assignable root cause was determined to be due to device wear from normal use and servicing over time.If additional information should become available, a supplemental medwatch report will be sent accordingly.
