The patient was undergoing a thrombectomy procedure using a penumbra system aspiration pump max 110v (pump max).During the procedure, it was reported that the pump max was not working and was ¿acting funny¿.Therefore, the procedure was completed using a new pump max.It is unknown if there was an adverse effect to the patient.Please note that the manufacturer has requested additional information from the customer; however, no additional information has been obtained.

Results: the vacuum gauge needle oscillated when the penumbra system aspiration pump max 110v (pump max) was generating vacuum.The accuracy of the vacuum gauge was within the specification.A small dent can be seen on the back of the pump max.Conclusions: evaluation of the device revealed the pump max vacuum gauge needle oscillated when the pump was generating vacuum.The accuracy of the vacuum gauge was tested, which calls for less than ±2 inhg total oscillation of the vacuum gauge when the pump is set to -25 inhg, and the pump max was found to be within specification.The pump max was able to power up repeatedly and produced consistent vacuum pressure.Penumbra pumps are visually and functionally inspected during incoming quality inspection.The manufacturing records for this lot were reviewed and did not reveal any outstanding discrepancies, design, or quality concerns.
