The insulin pump was unable to perform displacement test due to missing retainer.The pump was received with cracked case on battery tube area and minor scratches on lcd window.The pump was received with missing reservoir tube o-ring, cracked battery tube threads; scratches on display window cover, scratched casing, pillowing keypad overlay, cracked select button on keypad overlay, and damaged keypad overlay texture and peeling end cap address label.The insulin pump involved in this event is the 640g insulin infusion pump, which is not marketed in the united states.However, the device is similar to the paradigm real-time insulin infusion pump, which is marketed in the united states.(b)(4).
 
Event Description
It was reported of a long crack at the beginning of the top of the pump, all along the length of the battery tube case.The customer mentioned that the display window was worn and damaged.The customer's blood glucose reading was unknown.The insulin pump will be returned for analysis.
