Event Description
|
The customer reported via phone call that the bottom of the reservoir compartment was worn out and the drive support cap was indented.The customer's blood glucose was 10.9 mmol/l at the time of incident.The customer stated that the drive support cap was flush.The customer was advised that the insulin pump will need to be replaced.The customer was advised to revert to back-up plan.The insulin pump will not be returned for analysis.

The insulin pump was received with partially detached end cap.Drive support disk was inspected and no anomaly was noted.Device was received with cracked case at display window corner, cracked lcd window, minor scratched lcd window, cracked battery tube threads and cracked reservoir tube lip.
