On (b)(6) 2016, this 15 mm amplatzer septal occluder (aso) was implanted without difficulties.In (b)(6) 2016, a significant residual shunt was noticed and it was observed that the aso had embolized to the descending aorta.Tee showed that the aso is nicely orientated with blood flow and jammed just below the heart level.Per report, the patient was doing fine, although the significant asd remained which was closed with a larger 18 mm aso on (b)(6) 2017.The aso remains where it embolized.

Udi: (b)(4).The results of the investigation are inconclusive since the device was not returned for analysis.Our investigation was limited to the review of the device history record, which showed that each manufacturing and inspection operation was performed and indicated complete in accordance with sjm specifications and procedures.Based on the information received, the cause of the reported incident could not be conclusively determined.
