The companion 2 driver will be returned to syncardia for evaluation.The results of the evaluation will be provided in a follow-up mdr.(b)(4).
 
Event Description
The customer, a syncardia authorized distributor, reported that the companion 2 driver exhibited dropping fill volumes when hospital staff was connecting driver to wall air.The customer also reported that the patient was switched to a backup driver without any clinical impact.
 
Manufacturer Narrative
The companion 2 driver was returned to syncardia for evaluation.The electronic patient data were reviewed and confirmed drops in both left and right cardiac output and fill volumes.However, the fill volumes did not drop to zero as reported by the customer, but were recorded as low as 13cc (right fv) and 15cc (left fv).During investigation testing, a drop in fill volumes was reproduced but not to zero as was reported.The root cause of this drop in fill volume issue was identified as a malfunction of the pilot valves.Syncardia has an open corrective and preventive action (capa) to address this issue.Syncardia has completed its evaluation of this complaint and is closing this file.(b)(4) follow-up report 1.
