Manufacturer Narrative
The cause for the discordant ft4 results is the presence of anti-t4 antibody. This was confirmed by the customer. The customer sent the sample to a reference laboratory where the sample was run for the measurement of anti-t4 using a radioisotope method. It seems to be a qualitative method where it is considered a positive result when the percentage of fixation is above 10.For this patient sample, the percentage of fixation was 30%.The patient sample is not available for further testing. The instrument is performing within specifications. No further evaluation of the device is required. The ifu states in the interpretation of results section: "results of this assay should always be interpreted in conjunction with patient's medical history, clinical presentation and other findings.".

Event Description
A false high advia centaur xp ft4 result was obtained for a patient sample that did not match the tsh results. The patient sample was sent to a reference laboratory and tested with an alternate method. The result was lower.Patient treatment was not prescribed or altered. There was no report of adverse health consequences due to the discordant ft4 result.
