Event Description
|
It was reported to boston scientific corporation that a symphion fluid management accessories was used during a polypectomy procedure performed on (b)(6) 2017.According to the complainant, during preparation, the fluid management accessories was being set-up and the pressure sensor was not detected.The scope was disconnected from the controller several times and the controller was restarted twice and the pressure sensor was still not detected.A new fluid management accessory was used with the same scope and controller and the procedure was completed.There were no patient complications reported as a result of this event.The patient condition at the conclusion of the procedure was reported to be fine.

A symhpion device was returned for analysis.Visual analysis revealed no signs of damage to the device.Functional analysis revealed that the pressure sensor connector was possible to be inserted into the receptacle on the connector.However, the controller did not detect the pressure sensor.Therefore, the complaint was confirmed.Given the event description and condition of the returned device, there isn't enough information to determine a probable root cause.A review of the device history record (dhr) was performed; no anomalies were noted.A search of the complaint database revealed that no similar complaints exist for the specified lot.
