Event Description
Information was received from a manufacturer representative (rep) via a patient with an implantable neurostimulator (ins) for failed back surgery syndrome and spinal pain.The rep stated that the patient¿s charger kept beeping so it was replaced.The rep reset the charger but it continued to beep.The rep stated that the charger looked fine.The rep would return the charger.The issue was resolved at the time of the report and the patient was noted to be alive with no injury.There were no patient symptoms reported and no complications reported.Additional information received on (b)(6) 2017 from the patient reported that the insr freezing and having a blank screen/beeping has continued to occur despite working with the rep to reset it.It was also reported that the ins was taking too long to charge.It was noted that the patient usually gets 6-8 coupling bars shaded and typically starts recharging when ins is down to half, and that they keep the desktop charger plugged into the insr when charging.The patient was unable to troubleshoot while on the call due to an the issue with the insr.
