Event Description
|
It was reported that the lead was attempted to be implanted but not used.The lead was found to have low lead impedance.The physician noticed blood lodged throughout the inner insulation of the lead body.The physician felt uncomfortable with the drop in impedance and the blood inside the lead body and decided to use a different lead.The lead was removed from implant and a new lead was implanted.No patient complications have been reported as a result of this event.

Product event summary: the full lead was returned and analyzed.Analysis indicated that the inner insulation of the lead was extrinsically breached due to a cut.There was blood on the distal conductor of the lead and it was not obstructed.The outer insulation of the lead was extrinsically breached due to a cut.Visual summary analysis of the lead indicated damage at implant.The analyst confirmed, using destructive analysis, a cut through from the outer to the inner insulation and noted that blood was visible on distal conductor.If information is provided in the future, a supplemental report will be issued.
