(b)(4).As per field service representative (fsr), the system 1 tube clamps have a knob for opening each side of the clamp.The customer opened it up and found a broken piece of plastic.The fsr sent a replacement tube clamp to the customer.

|
It was reported that during set-up of the device for a cardiopulmonary bypass (cpb) procedure, the roller pump tube clamp was stuck and the knob would not turn.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.

During laboratory analysis, the product surveillance technician (pst) could not rotate the tube clamp knob.Internal inspection revealed a broken cam on left lower slide affecting proper operation of the tube clamp assembly.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
