The sample was not returned for evaluation; therefore, the complaint was not confirmed and a root cause was not able to be determined.A retention sample from the same product code/lot number combination was obtained.Visual inspection was performed on the retention sample, during which no anomalies were noted.Ops valves are subject to a 100% leak test, which includes five different tests the units are run through to ensure there are no leaks and both the umbrellas and duckbills are functioning properly.This issue is currently undergoing an investigation; however, it is presently believed that the lack of flow through the duckbill valve is due to a process change in the water that the duckbills are washed in at the supplier.(b)(4).All available information has been placed on file in quality management for appropriate tracking, trending and follow-up.
 
Event Description
The user facility reported to terumo cardiovascular that during setup, the one way valve did not open up immediately on its own.Per sales representative, the customer followed the instructions on the letter received regarding the valves and made it open.They were able to use it throughout the surgery with no issues and problems.No known impact or consequence to patient.Product was not changed out.Procedure was completed successfully.
