Event Description
It was reported that during an unspecified surgical procedure, it was observed that the motor device did not work.There were no delays to the surgical procedure as a spare device was available to complete the surgery successfully.There was patient involvement reported.There were no reports of injuries, medical intervention or prolonged hospitalization.All available information has been disclosed.If additional information should become available, a supplemental medwatch report will be submitted accordingly.

The actual device has been returned and is currently pending evaluation.Once reliability engineering evaluates the device, a supplemental medwatch report will be sent accordingly.

The actual device was returned for evaluation.Reliability engineering evaluated the device and the reported condition of the device not working was not confirmed.Therefore, an assignable root cause was not determined.However, during evaluation, the device was tested and it was noted that the connector shell was not secured.The connectors were examined and photographed using 40x magnification and it was determined that the red loctite appeared to be present and evenly distributed.It determined that the root cause of the improper loctite application was due to improper assembly of the device during the manufacturing process.This issue has been escalated to capa.If additional information should become available, a supplemental medwatch report will be submitted accordingly.
