Event Description
|
It was reported that when the protective sheath was removed from the 2.5 x 23 mm xience alpine stent delivery system it felt stiff and pulled the stent off the balloon with it.There was no resistance noted during removal of the protective sheath.The device could not be used on the patient.A new stent delivery system was used to complete the case.There was no clinically significant delay in the procedure reported.No additional information was provided.

(b)(4).The device was returned for analysis.The reported stent dislodgement was able to be confirmed.The reported physical property issue was unable to be replicated in a testing environment as it was based on operational circumstances.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents from this lot.The investigation determined the reported difficulties appear to be related to circumstances of the procedure as it is likely that inadvertent mishandling resulted in the noted bent protective sheath thus as the sheath was being removed resulted in the reported physical property issue/irregular texture.Manipulation to remove the sheath resulted in the noted stent damages (bent, crushed, flared) and ultimately resulted in the reported stent dislodgement.There is no indication of a product quality issue with respect to manufacture, design or labeling.
