This alleged failure mode poses a low risk to the patient because although the freedom onboard battery did not work, patients are provided with several onboard batteries, and the freedom driver has a redundant power source of external wall power.The freedom onboard battery has been returned to syncardia for evaluation.The results of the investigation will be provided in a follow-up mdr.(b)(4).

The customer, a syncardia certified hospital, reported that one of the patient's freedom onboard batteries does not work.There was no reported patient impact.

|
The cause of the customer-reported battery not providing output was the result of battery exhibiting a permanent fault.The root cause for the battery's disabled output cannot be conclusively determined; however, analysis of the system management (smbus) recorded data indicated that two of its cells were at zero volts.The battery's cells were damaged to the extent that a cell imbalance (cim) permanent fault condition was detected, causing the safety monitor circuitry to permanently disable the battery's input / output functions.This issue will continue to be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation and is closing this file.(b)(4) follow-up report 1.
