Event Description
|
It was reported that the patient underwent posterior lumbar interbody fusion at l2/5 due to lumbar spinal canal stenosis.Intra-operatively, the cage broke on insertion into l3/4 when it was struck two or three times.The upper surface of the cage broke when about half of the cage was inserted to the intervertebral space.The broken part was removed and the surgery was continued with another cage after confirming no fragments in the body.No patient complications were reported as a result of this event.

Product analysis: the spacer has fractured at one of the locating tabs that align the inserter.The fracture appears to be the result of the inserter contacting the lower wall of this cutout and overloading the material.If information is provided in the future, a supplemental report will be issued.
