Boston scientific received information that the patient was implanted with this cardiac resynchronization therapy defibrillator (crt-d) and right ventricular (rv) lead.After the pocket was closed, additional testing was performed and the scrub technician applied pressure on the pocket.Oversensing was found.An x-ray taken was reviewed and the terminal pin of the lead appeared seated appropriately in the header.The pocket was then reopened and the lead was reconnected.There was some fluid that came out of the seal plug, therefore air was suspected.No oversensing was noted after reconnecting the lead to the header.The pocket was reclosed and no further issues have been reported.
