This alleged failure mode poses a low risk to a patient because the issue was observed when the companion 2 driver was not supporting at patient.The companion 2 driver has been returned to syncardia for evaluation.The results of the evaluation will be provided in a follow-up mdr.(b)(4).
 
Event Description
The customer, a syncardia authorized distributor, reported that the key was found broken off inside the key switch of the companion 2 driver.
 
Manufacturer Narrative
The companion 2 driver was returned to syncardia for evaluation.Visual inspection of the driver revealed a broken key inside the key switch, confirming the customer-reported issue.Other signs of rough handling were also observed and documented as part of this investigation.Although the key was broken inside the key switch, the switch was still operational and the driver passed incoming functional testing.The root cause of the broken key could not be conclusively determined; however, it is known that the key broke sometime after leaving syncardia and was most likely the result of rough handling.This issue will continue to be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation of this complaint and is closing this file.(b)(4) follow-up report 1.
