Event Description
Information was received from a healthcare professional via a company representative regarding a patient receiving an unknown medication via an implanted pump.The indication for pump use was malignant pain/cancer pain.On (b)(6) 2018 it was reported that the patient¿s pump was not able to be read.They had tried with three different physician programmers.No patient symptoms were reported.Additional information was received on 14-mar-2018 from a company representative who reported that the physician had no updates regarding the issue.Additional information was received on 26-mar-2018 from a company representative who reported that the physician had been asked a few times as to what was happening with the patient, but no further information had been received.No further complications have been reported as a result of this event.

(b)(4).Review of the additional information received has determined that there is no information to reasonably suggest that the device in this report may have caused or contributed to a death or serious injury or that the device in this report has malfunctioned.Therefore, this event no longer meets the reporting requirements stipulated in 21 cfr 803.Medtronic, inc.If information is provided in the future, a supplemental report will be issued.
 
Event Description
Additional information was received on 30-mar-2018 from a company representative who reported that the pump was able to be read with the company representative¿s programmer in a different environment.There were no concerns for the patient or the pump at this time as result.The issue was resolved.No further complications were reported/anticipated.
