(b)(4).The device was not returned for evaluation.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.The reported patient effects of ischemia and thrombosis are listed in the supera instructions for use as known patient effects associated with the use of the device.Based on the case information and related record review, a conclusive cause for the reported patient effect(s), and the relationship to the product, if any, cannot be determined.Based on the information reviewed, there is no indication of a product quality issue with respect to manufacture, design or labeling.The additional supera is being filed under a separate medwatch report.
 
Event Description
It was reported that the procedure was to treat the femoral vein below bifurcation that was moderately tortuous and moderately calcified.Two supera peripheral stents were implanted successfully (supera veritas 5 x 200 and supera 5.5 x 80).Another non-abbott vascular stent was implanted and blood flow was 'jagged' (slow flow).The physician mentioned probable cause was spasm and thrombus which became evident during the procedure.Thrombosis was confirmed via angiography.It was confirmed all three stents were post-dilated with improved blood flow and thrombus moved more distally.Patient received a treatment dose of dalteparin the same day as aspirin and clopidogrel.Patient was kept overnight for observation and the next day doppler showed patient arteries with no thrombus.No additional information was provided.
