It was reported that the right ventricular (rv) lead exhibited high and fluctuating impedance on the high voltage coil. When the lead was explanted and tested with an analyzer noised was also noted. At the midpoint of the lead an insulation breach was intentionally created to facilitate the introduction of a new lead. The lead was successfully explanted and replaced. No patient complications have been reported as a result of this event.

Manufacturer Narrative
Product event summary: the full lead was returned and analyzed.Analysis indicated that the right ventricular defibrillation coil developed a fracture due to flexing while in vivo. The overlay tubing and the outer insulation of the lead was extrinsically breached due to a cut.If information is provided in the future, a supplemental report will be issued.
