Event Description
It was reported that a hair was noticed inside the sterile pouch of the 014 balance middleweight (bmw) hydro guide wire.The sterile pouch was not opened.This was found while still in the purchasing office.The guide wire was not used in a procedure.No additional information was provided.
 
Manufacturer Narrative
(b)(4).The device was returned for analysis.The reported foreign material was able to be confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents from this lot.The investigation determined the reported difficulties appears to be related to a potential product quality issue.The issue is being addressed per internal operating procedures.Abbott vascular will continue to trend the performance of these devices.Four sterile/unused guide wires were returned from the account.The four sterile guide wires were visually inspected.No foreign material was noted inside the pouches or on the guide wires.
