Concomitant medical product: erbe electrosurgical generator.Investigation evaluation: a product evaluation was not performed in response to this report because the product said to be involved was not provided to cook for evaluation.The report could not be confirmed.The device history record for the lot number said to be involved could not be reviewed because a lot number was not provided.Investigation conclusion: a corrective action has been initiated to reduce occurrences of stripping difficulty for fs-omni sphincterotomes.The product said to be involved is included in the scope of the corrective actions.Prior to distribution, all fusion omni-tome pre-loaded sphincterotomes are subjected to a visual inspection and functional test to ensure device integrity.Corrective action: a review of the complaint history was conducted.A corrective action has been initiated in an effort to reduce occurrences of this nature.This product is included in the scope of this corrective action.Quality assurance will continue to monitor for complaint trends and reassess the risk assessment results as post market feedback continues to become available.
 
Event Description
During an endoscopic retrograde cholangiopancreatography (ercp), the physician used a cook fusion omni-tome pre-loaded sphincterotome.The wire did not split out of the catheter properly.It was really difficult.[there was a] "stripping issue" with wire [guide] and plastic [catheter] at the time they were ready to make a device exchange for a balloon [difficult exchange].The problem was that the wire was not splitting properly away from the sphincterotome catheter and the customer lost access and positioning [lost wire guide access] (subject of this report).The case was aborted as it was a very challenging patient and anatomy.
