This alleged failure mode poses a low risk to the patient because although air may have been leaking from the red cpc connector on the driveline of the freedom driver, the freedom driver continued to perform its life-sustaining functions.The freedom driver has been returned to syncardia for evaluation.The results of the investigation will be provided in a follow-up mdr.(b)(4).
 
Event Description
The cpc (colder products company) connector is a component that provides the interface between the driver drivelines and the tah-t cannula.The customer, a syncardia certified hospital, reported that the patient stated he could feel "air leaking" at the red cpc connector.The hospital staff reported that they were unable to feel such a leak but did note they could hear air movement at the red cpc connector.There was no reported adverse patient impact.
 
Manufacturer Narrative
The customer-reported air leak in the red (left) cpc (colder products company) connector was not able to be reproduced or confirmed during investigation testing; therefore, a root cause for the reported air leak could not be conclusively determined.Visual inspection of the driver, including the drivelines and both cpc connectors, revealed no abnormalities, and the driver passed all incoming functional test requirements.This issue will continue to be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation and is closing this file.(b)(4) follow-up report 1.
