Event Description
A surgeon reported that she has had multiple patients in which she has had retained fibers post surgery found on first day post-op.She had been seeing fibers post operatively since august on and off ( no distinguishing patient identifiers or date of events available).There has been no patient harm.She has just recently seen an increase over the past month.Additional information was provided by the operating room manager that there are two sets of patients: one set that the fiber was discovered during surgery and another set that the fiber was discovered on post op day 1.There are at least 10 patients in which the fiber was discovered on post op day 1.All 10 had the fiber removed as it was in the incision site.The surgeon is still unsure of where the fiber is from so is still considering that they could have come from the cartridge.Another medical device report has been submitted for the patients with fibers removed during surgery.This report is for the patients that had the fibers removed on post op day 1.
