Event Description
The reporter indicated that before the surgeon implanted a 12.6mm vticm5_12.6, -10.5/+2.0/087 (sphere/cylindr/axis), implantable collamer lens into the patient's right eye (od), he noticed a foreign body on the lens.Reportedly, a thread-shaped foreign body was found on the lens during loading.The lens did not contact the eye.This occurred on (b)(6) 2018.
 
Manufacturer Narrative
Device evaluation: lens was returned dry in a micro-centrifuge vial with surgical residue on product.Visual inspection found haptic torn with torn piece missing and foreign material (residue) on lens surface.It should be noted that no foreign body was noted on lens.Claim # (b)(4).
