No patient information provided as no patient was involved in this concern.Unique device identifier (udi) is unavailable.Device manufacture date is unavailable.A medtronic representative went to the site to test the equipment.Testing revealed that the 400v were not present on the outbound side of the power enclosure.Replacing the enclosure resolved the issue.The imaging system then passed the system checkout and was found to be fully functional.The power enclosure has been received by the manufacturer for evaluation however results are not available yet.
 
Event Description
Medtronic received information regarding an imaging device being used outside of procedure.It was reported that after the imaging system was installing, the system was stuck in the initializing phase.There was a loss of 400 v out of the power conversion enclosure.There was no patient involvement.
 
Manufacturer Narrative
The analysis on the power conversion enclosure was completed by medtronic personnel.Testing found that a transistor on the circuit board of the unit was shorted.
