No patient information provided as no patient was involved in this concern.A medtronic representative went to the site to test the equipment.The transformer and the power cable assembly were replaced.The hardware, software, and instruments passed the system checkout.The system was found to be fully functional.The transformer and the power cable assembly were returned to the manufacturer for analysis.No fault was found with the returned parts.
 
Event Description
Medtronic received information regarding a navigation system.It was reported that the system rebooted on its own on the ¿select patient¿ screen.When the system booted back up, it went through the entire booting up process and then rebooted on its own again.It happened three times and then the system displayed ¿no sync¿ in green led light, the screen went black with orange led, and then the led turned off and the system has been black since.The computer and monitor have already been replaced.No patient was present at the time of the event.
 
Manufacturer Narrative
Correction: medtronic received information that replacing the power switch did not resolve the issue.Additional information: however, the system has been replaced and is no longer at the site.
