No patient information provided as no patient was involved in this concern.A medtronic representative confirmed the issue and reported that the dongle would not attach to the panel and found that the standoffs were damaged.The broken screw shafts from mounting cable were removed.The representative reported that standoffs were replaced and dongle was reattached to resolve the issue.The rear cab breakout panel was returned to the manufacturer for evaluation.Through visual and physical testing it was confirmed that two standoffs screws were missing from the replacement.The standoff screws were removed from the replacement rear cab breakout panel and put into the panel that had the broken standoff screws in order to fix the problem.
 
Event Description
Medtronic received information regarding an imaging system.It was reported that the imaging system was booting but was stuck on e-stop.While troubleshooting it was found that the dongle was disconnected.When the dongle was reset, the e-stop was able to be cleared however, the dongle appeared to have been bumped and would not stay seated in the screws.No patient was present.
