A medtronic representative went to the site to test the equipment.Testing revealed that the representative was unable to replicate the reported issue.It was reported that the mobile view station (mvs) remained unplugged from the image acquisition system (ias) for the remainder of the case and when the viewing station was re-plugged into image acquisition system (ias), the system booted up fully and the lights and text could be seen.The imaging system then passed the system checkout and was found to be fully functional.
 
Event Description
Medtronic received information regarding a imaging system being used for a electrode and probe placement.It was reported that the pendant did not have power.The lights were on and nothing was showing on the screen.The site tried rebooting the system, but the issue persisted.During initial troubleshooting, it was reported that a damage to the key was noticed.It was loose and there was damage to the dongle and on the plastic on the umbilical connector.Imaging was aborted, and the procedure was extended less than 1 hour.Site used a c-arm to continue with procedure.No known impact on patient outcome.
 
Manufacturer Narrative
It was reported that the key switch and dongle on the imaging system were repaired by medtronic personnel and did not require replacement.
