Event Description
Medtronic received information regarding a navigation device being used for a sacroiliac and thoracolumbar fusion procedure.It was reported that preoperatively with no patient present, the site attempted to start the navigation system but failed as the system indicated ¿no signal.¿ several restarts of the system were attempted.The navigation system eventually started but worked in an unstable manner and was noted to be unresponsive.Thus, use of the system was discontinued prior to being used in the procedure.Instead the procedure was completed with the use of a c-arm.There was no surgical delay due to the reported issue, and there was no known impact on patient outcome.
 
Manufacturer Narrative
A medtronic representative went to the site to test the equipment.Testing revealed that the computer of the navigation system was replaced to restore functionality.The system then passed the system checkout and was found to be fully functional.The computer for the navigation system was returned to the manufacturer for analysis.The computer was found to be fully functional with no problem found.The reported event could not be duplicated by medtronic personnel.
