A medtronic representative went to the site to test the equipment.Testing revealed that the imaigng system was booting in stand alone mode and logs shoe detector interface connection not optimal.Replacing the interface cable resolved the issue.The imaging system then passed the system checkout and was found to be fully functional.The interface cable has not been received by the manufacturer for evaluation.
 
Event Description
Medtronic received information regarding an imaging system.It was reported intra/peri-operatively during the select patient/acquire scans task of an electrode and probe placement procedure, the system would not move out of standalone mode.The site rebooted the system while the umbilical cable connected with no change.They then rebooted the system while the umbilical cable was disconnected, and then once both systems were booted they connected the umbilical (did this twice) with no resolution.This caused a 45-minute delay before site chose to abort use of the imaging system and the take patient for a ct.There was no reported impact on patient outcome.
 
Manufacturer Narrative
The interface (umbilical) cable was returned to the manufacturer for evaluation.Testing found that the board had two opens in associated pins.
