Event Description
The customer's nurse reported via phone calk that they went to the emergency room on (b)(6) 2018 by emergency medical services due to the insulin pump not working.The nurse stated the insulin pump's buttons are hard to press and would like a replacement for the pump.Customer's blood glucose level was 112 mg/dl.Customer was advised to discontinue use of the device and revert to a backup plan.The customer was advised that the device would be replaced and agreed to return the product for analysis.

The insulin pump was received with intermittent button response due to flattened button dome switch (no crease) and partial unlocked keypad connector noted during visual inspect.Unable to perform all functional testing including the displacement, operating currents, unexpected restart error test, rewind, basic occlusion, occlusion, prime and excessive no delivery test due to buttons not responding.
