The insulin pump was received with the following physical damage: faded serial number label, cracked battery tube threads, partially broken reservoir tube lip, missing reservoir tube retainer, missing reservoir tube o-ring, minor scratched lcd window and cracked select button keypad overlay.The insulin pump involved in this event is the paradigm real-time 640g insulin infusion pump, which is not marketed in the united states.However, the device is similar to the paradigm real-time insulin infusion pump, which is marketed in the united states.

It was reported that the insulin pump's serial number had worn out.Blood glucose level at the time of the incident was 5 mmol/l.No further details were given.The insulin pump was returned for analysis.
