Patient information was unavailable from the site.A medtronic representative went to the site to test the equipment.The field service engineer (fse) could not duplicate this reported event.Review of the log files did not show anything out of the ordinary.The imaging system passed the system checkout and was found to be fully functional.However, the fse found ethernet cables on o-arm and stealth to be mangled and taped up due to damage.This is was determined to be the problem.The site needed to order new cables to resolve this issue.
 
Event Description
Medtronic received information regarding an imaging system being used during a sacroiliac and thoracolumbar procedure.It was reported that the imaging system was note taking any images in a case the previous day.It was not connecting to the navigation system either.The issue resolved after three reboots.Issue occurred intra/peri-operatively with less than one hour delay to surgical time.There was no reported impact to patient outcome.
