It was reported that during set-up of the device for a cardiopulmonary bypass (cpb) procedure, the coating around the sensors was gone.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.

The reported complaint was confirmed.The complaint was confirmed by pictures that the user facility sent of the damaged level sensor to the manufacturer but the level sensor was disposed of so no further evaluation will be conducted.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
