Event Description
|
The freedom home ac power supply is a component that enables the freedom driver to be plugged into an external power source.The customer, a syncardia certified hospital, reported that the patient turned in a freedom home ac power supply with a chip in the connector.The customer also reported that the cord works but the patient wanted it replaced.The customer also reported that there was no adverse patient impact.

The visual inspection confirmed the customer-reported issue of a broken white connector, while also identifying a missing foot on the ac power supply brick.The functional inspection found the observed damage to be solely cosmetic, as the ac power supply passed all other sections of testing.The root cause of the broken white connector could not be conclusively determined but is likely the result of either rough handling or wear from usage.This issue will continue to be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation and is closing this file.(b)(4) follow-up report 1.
