Event Description
Complaint description: patient was revised to address metallosis.Update: asr litigation record received.Litigation alleges pain, stiffness, discomfort, weakness, mobility problem and quality of life, anxiety, toxicity of the metal in the body and emotional distress.Plaintiff alleges three dislocation but there is no information if depuy products were involved.Doi: (b)(6) 2007; dor: (b)(6) 2016; left hip.

> the asr platform was voluntarily recalled from the market in august 2010, and the asr product codes are now considered inactive.Further investigation of this individual incident will not be undertaken.The investigation regarding the root cause(s) and/or corrective actions was conducted under mdd capa- (b)(4).Ongoing post market surveillance is conducted per our procedures for this product.
 
Event Description
After review of medical records, patient was revised to address failed asr.Revision notes reported of black tinged synovitis most likely metallosis which were all excised meticulously to minimize post -operative residual inflammation and pain.
