This alleged failure mode poses a low risk to the patient because the reported issue did not prevent the freedom driver from performing its life-sustaining functions.In addition, the freedom driver has a redundant power source of multiple onboard batteries.The freedom home ac power supply will be returned to syncardia for evaluation.The results of the investigation will be provided in a follow-up mdr.(b)(4).

The freedom home ac power supply is a component that enables the freedom driver to be plugged into an external power source.The customer, a syncardia certified hospital, reported that the patient's freedom home ac power supply had a loose connection.The customer also reported that the ac power supply was exchanged, and there was no adverse patient impact.

The customer-reported issue, a loose connection, was confirmed during visual inspection and functional testing.Visual inspection revealed a broken hypertronics connector.Functional testing determined that the power supply was operating correctly but had an intermittent contact with the driver causing the unit to not pass functional testing requirements.The root cause of the reported issue cannot be conclusively determined but is likely the result of a drop or other rough handling of the ac power supply.This issue will continue to be monitored and trended as part of the customer experience process.Syncardia has completed its evaluation and is closing this file.Ce 4551 follow-up report 1.
