Event Description
It was reported to boston scientific corporation that an obtryx system - halo was used during an hta + sling + bladder "rafia" procedure performed on (b)(6) 2018.According to the complainant, during the procedure, the association loop detached from the device during the deployment of the device through the patient's left side.Reportedly, the physician was informed about the need to remove the detached piece from the patient, but after several attempts, it was unsuccessful.It was then decided by the patient to leave it in place and evaluate it later.Attempts to obtain additional information regarding the circumstances surrounding this event have been unsuccessful to date.Should additional relevant details become available, a supplemental report will be submitted.
