Event Description
|
Information was received from a manufacturing representative (rep) regarding a patient implanted with a neurostimulator (ins).The rep reported that they were called in to a case for a lead revision on (b)(6).When the healthcare professional (hcp) opened the pocket, the lead slid out of the ins.The hcp used another ins and connected to the lead already there, impedance tested fine and the hcp closed the pocket.The patient was getting good therapy after.No further complications were reported or are anticipated.

Analysis of the implantable neurostimulator (ins) model 3058, serial # (b)(4), showed no significant anomalies.It was reported that the set screw was backed out too far.It was noted that the device was subjected to a series of standard tests that included (but was limited to) visual inspection, output, and telemetry.Good, stable out was seen on all electrode pairs.If information is provided in the future, a supplemental report will be issued.
