Event Description
It was reported that bd posiflush¿ ns filled syringe stopped pushing during a flush.This occurred on 4 separate occasions.No serious injury or medical intervention was reported.Verbatim: "material no.: 306546 batch no.: 8197649.It wa reported that the pre-filled syringe stopped pushing during a flush.Per complaint form: during the flushing process in pet/ct unit, about 5 bd prefilled 10cc saline syringes were encountered that stopped pushing before the syringe was empty.While pushing in the flush, they were able to get in about 5 or 6 cc¿s, and then it stopped.They weren¿t able to push anymore flush in.It locked up and they weren¿t able to push or pull the plunger in either direction.".
