It was reported that on (b)(6) 2019, a k-wire was stuck in a hole of an insertion guide.It is unknown if there was a surgical delay.Procedure outcome and patient status was unknown.This complaint involves two (2) devices.This report is for one (1) insertion guide.This report is 1 of 2 for (b)(4).

Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.H3, h4, h6- service and repair evaluation the customer reported the that on (b)(6) 2019, a k-wire was stuck in a hole of an insertion guide.The repair technician reported that the screw (27h) was missing.Missing parts is the reason for repair.The cause of the issue is unknown.The following parts were replaced: replacement screw for insertion guide (27h) the item was repaired per the inspection sheet, passed synthes final inspection on 24-jun-2019 and will be returned to the customer upon completion of the service and repair process.Attached service record router completed through operation 30.Finalized service record will be archived in tungsten document management system.The evaluation was confirmed.The device was deemed serviceable and will be returned to the customer, no design or manufacturing issues were identified therefore it has been determined that no corrective and/or preventative action is proposed.Device history lot part #: 323.050 , lot #: 1894632 , manufacturing site:hägendorf, release to warehouse date: 28.May 2008.A manufacturing record evaluation was performed for the finished device lot number, and no non-conformances were identified.Device was used for treatment, not diagnosis.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
