Event Description
According to the reporter, during an esophageal anastomosis with small intestine procedure, the device had no stapler.The surgeon then used another device to resolve the issue in order to complete the case.There was no patient injury.

Event Description
According to the reporter, during an esophageal anastomosis with small intestine procedure, the device staples did not deploy.An unanticipated tissue loss and tissue damage occurred as a result of the tissue damage.The surgeon then used another device to resolve the issue in order to complete the case.
