Manufacturer Narrative
Date of event: (b)(6) 2018.Date of report: 26aug2019.The manufacturer's field service engineer (fse) confirmed the reported electrical burnt smell issue. The customer replaced the defective power supply to address the reported problem. The power supply, 3rd generation, v200 was returned to failure investigator (fi) lab for evaluation.Visual inspection of this power supply did not reveal any signs of damage or contamination. This power supply will be installed into the fi test ventilator and an attempt to boot into the normal ventilation mode will be made while operating on the ac source. The ac power led did activate when power was applied.Cycling the power on and off did not produce any errors.Final results: the test ventilator did boot up and deliver breaths and the test ventilator did not generate any errors during approximately 4 hours of operation.Mdr has been reassessed as reportable after a request from the fda on march 1, 2019 to review complaints from 29 nov 2017 to 29 nov 2018.As this has been reassessed, it will appear to be a late mdr.
